Program Coordinator - Full-Time Position in Norwalk, OH, Office

Catholic Charities – Diocese of Toledo is seeking a full-time Program Coordinator for the Norwalk locations.

Position Description:

The Program Coordinator leads the effective and efficient delivery of ministry services to Miriam House guests in support of Catholic Charities' purpose, mission, and core values. Through consistent training, the coordinator ensures that Residential Specialists and the Community Health Worker understand their specific work responsibilities, priorities, and purpose within the organization.                                       

Essential Responsibilities:       

  • Provides leadership to ministry personnel through daily communication, ongoing performance feedback, information sharing, ongoing training, development, and hands-on leadership.                                                                

  • The Program Coordinator is a key contributor in ministry planning, budgeting, goal setting, and grant application input.

  • Establishes and maintains community connections in support of ministry objectives. 

  • Participates in community forums, workshops, and meetings impacting ministry services.  
  • Shares timely communication of ministry events, volunteer activities, and client transformation stories with the Program Director. 
  • Responsible for overall leadership of the operations and activities of the Community Emergency Services ministry, which delivers crisis navigation services, financial assistance for basic needs, financial literacy, and case management services to include, but are not limited to, information and referrals, client intake and assessment, 1-1 financial counseling/workshops, ongoing case management, and, if desired by clients, offers spiritual engagement.  
  • Works with local employers and community partners to expand opportunities for individuals.
  • Conducts periodic home visits with clients along with the Community Health Worker.
  • Responsible for ensuring client case documentation is timely and complete and adheres to federal, state, and local regulations.
  • Seeks out collaboration opportunities with relevant community partners.
  • Performs other related duties, as assigned by the supervisor.

 Candidates for the position must possess:                                                                                                          

  • Bachelor's degree from accredited college or university in Business, Social Work, Psychology, Healthcare, or a similar field. 

  • Three years of relevant work experience; supervisory experience is preferred.                                                          

  • Maintain a current driver's license and the ability to travel within the Diocese of Toledo.                             

  • Computer-savvy and proficient with Microsoft Excel, Word, and PowerPoint.                                                      

  • Ability to work flexible hours; some evening and weekend hours may be necessary.                     

  • Successful BCI/FBI background check, prior to employment, and every 5 years.

Case Manager - Full-Time Position in Mansfield, OH, Office

Catholic Charities - Diocese of Toledo is seeking a full-time Case Manager for its direct service ministries in Mansfield, Ohio.

Position Objective

The Case Manager is responsible for helping stabilize families and individuals in crisis by providing resources and support to educate, empower, and encourage them in a safe, predictable environment. They will work directly with clients to develop and implement an individual service plan, based on their specific needs. May be required to obtain Community Health Worker certification and maintain caseload upon receiving same.

Essential Functions

  • The case manager will conduct an initial assessment of the client's needs, coordinate services, empower individuals and families to overcome barriers to achieving personal and professional goals, and develop strategies to address them.
  • Works closely with the clients to provide ongoing help in navigating available services and resources to meet their needs and offers appropriate referrals. Maintains regular contact with clients to track progress, adjust plans, and provide ongoing support. Acts as an advocate for client needs in the community.
  • Assists clients in developing and working toward personal goals to help them gain independence. These goals may include creating a budget, searching for a job, job preparedness, completing an education, attending life skills workshops/training, maintaining healthy relationships, or seeking spiritual growth.
  • Connects clients with community resources, including housing, healthcare, mental health services, employment programs, and benefits assistance.
  • Keeps accurate case files, record client interactions, and report outcomes to supervisors in accordance with program and related funding requirements. Case files must be kept current and well-organized.
  • Enters client data in applicable client tracking databases and tracks and reports appropriate data for monthly statistics, grants, and other reporting needs.
  • Assists the Program Coordinator in maintaining client statistics and completing activity reports.
  • Teaches life skills, mentors clients, and advocates for client needs within the community or with partner organizations. The program might require periodic home visits. 
  • Each location may have additional specific responsibilities within local case management services.                                                                               

Qualifications                                                                                  

  • Bachelor's degree from an accredited college or university, in Social Work, Psychology, or Sociology, with 1-3 years of relevant work experience in social services, case management, or working with vulnerable populations is highly valued.                       
  • Proficiency with Microsoft Excel and Word, and knowledge of local community resources.                                                  
  • Maintain a current driver's license and be able to travel within the Diocese of Toledo.

Residential Specialist - Part-Time at La Posada Family Emergency Shelter, Toledo, OH

Catholic Charities-Diocese of Toledo is seeking a part-time Residential Specialist for the La Posada Family Emergency Shelter in Toledo, Ohio. As a 24-hour operation, this part-time Residential Specialist must be available to work nights and weekends on a scheduled basis at the facility.

Position Objective                                                               

The Residential Specialist is responsible for maintaining a positive living environment for residents staying in the facility. They ensure the ongoing order, cleanliness, and safety of the facility are sustained. They provide customer-friendly service to our residents while ensuring all follow house policies and procedures.

Essential Responsibilities                                                                

  • Completes shift duties and responds to residents' needs, providing customer-friendly service.
  • Ensures our residents follow house policies and procedures, maintain their daily schedules, and complete assigned responsibilities (such as laundry and medication maintenance).
  • Completes administrative duties, including, but not limited to, answering phones and greeting visitors in a courteous and friendly manner, creating a weekly menu, grocery list, and schedule of responsibilities for residents.
  • Maintain activity logs and reports. Communicate necessary information to the Case Manager and Program Coordinator in a timely and clear manner.
  • Upon resident arrival, assists with moving in. Upon resident departure, cleans and prepares rooms for future resident occupancy.
  • Manage and assist residents with periodic house meetings, house activities, and workshops.                                                 

Qualifications                                                                     

  • High school diploma required or general equivalency degree (GED).
  • At least 1 year of relevant work experience in a social services environment is desirable.
  • Must have (or be willing to obtain) and maintain CPR and First Aid certifications.
  • Successful BCI/FBI background check, prior to employment, and every 5 years.
