Case Manager - Full-Time, Permanent Supportive Housing, Toledo, OH

The Case Manager is responsible for helping stabilize families and individuals in crisis by providing resources and support to educate, empower, and encourage them in a safe, predictable environment. They will work directly with clients to develop and implement an individual service plan, based on their specific needs. May be required to obtain Community Health Worker certification and maintain a caseload upon receiving the same.

Essential functions

• The case manager will conduct an initial assessment of the client's needs, coordinate services, empower individuals and families to overcome barriers to achieving personal and professional goals, and develop strategies to address them.
• Works closely with clients to provide ongoing support in navigating available services and resources to meet their needs and offers appropriate referrals. Maintains regular contact with clients to track progress, adjust plans, and provide ongoing support. Acts as an advocate for client needs in the community.
• Assist clients in developing and working toward personal goals to help them gain independence. These goals may include creating a budget, searching for a job, preparing for a job, completing an education, attending life skills workshops/training, maintaining healthy relationships, or seeking spiritual growth.
• Connect clients with community resources, including housing, healthcare, mental health services, employment programs, and benefits assistance.
• Keep accurate case files, record client interactions, and report outcomes to supervisors in accordance with program and related funding requirements. Case files must be kept current and well-organized.
• Enter client data into applicable client-tracking databases and track and report relevant data for monthly statistics, grants, and other reporting needs.
• Assists the Program Coordinator in maintaining client statistics and completing activity reports.
• Teaches life skills, mentors clients, and advocates for client needs within the community or with partner organizations. The program might require periodic home visits. Each location may have additional specific responsibilities within local case management services.
• Performs other related duties, as assigned by the supervisor.

Qualifications

• Bachelor's degree from an accredited college or university in Social Work, Psychology, or Sociology, with 1-3 years of relevant work experience in social services, case management, or working with vulnerable populations, is highly valued
• Proficiency with Microsoft Excel and Word, and knowledge of local community resources.
• Maintain a current driver's license, with the ability to travel within the Diocese of Toledo.
• Successful BCI/FBI background check before employment and every 5 years.

Facilities Assistant - Part-Time at Helping Hands of St. Louis (HHSL), Toledo, OH

The part-time Facilities Assistant supports HHSL's daily operations by assisting maintenance personnel in keeping the facility clean and safe and by handling and organizing donated food and supplies. This employee will assist with routine facility upkeep, support the Kitchen Manager, and manage the logistics of obtaining donations throughout Toledo.

Essential Responsibilities                                                                  

• Performs pick-up of supplies, equipment, donations, etc., in a ministry-supplied vehicle, as needed for the Toledo area Catholic Charities ministries
• Responsible for safely loading, transporting, and unloading donations.
• Assist the Kitchen Manager in stocking shelves, storing incoming food items, and maintaining an organized storage area.
• Rotate food inventory by removing older items and properly storing newly received items (first-in, first-out method).
• Follow all food safety, sanitation, and safety procedures as directed by OSHA.
• Clean and sanitize restrooms regularly to maintain a hygienic environment. Mop floors and assist with general cleaning throughout the facility.
• Assist with minor repairs, moving equipment, and general facility upkeep.
• Ensures that appropriate safety precautions are taken to protect self and others when completing required tasks.
• Completes maintenance and repair work in a timely manner.
• In cooperation with the maintenance lead, is responsible for all exterior property upkeep, including, but not limited to, lawn maintenance, snow removal, and general property cleanliness.     
• Reports safety hazards and property damage to the Program Coordinator in a timely manner.
• Assists with facility security as needed, when guests are present.
• Assists with moving boxes, cabinets, furniture, and other related office equipment.
• Maintains solid lines of communication with the Program Coordinators, including responding to emergency calls.
• Performs above job duties in accordance with OSHA safety standards.
• Performs other related duties, as assigned by the supervisor.                                             

Qualifications                                                             

• High school diploma required or general equivalency degree (GED), with at least 2 years of experience in facilities and equipment repairs and maintenance.
• Working knowledge used in general repairs and maintenance, including, but not limited to, plumbing, carpentry, glass replacement, painting, and grounds maintenance.
• Ability to safely operate small power equipment and tools.
• Must have and maintain CPR and First Aid certifications.
• Maintain a current driver's license and be able to travel throughout the Diocese of Toledo
• Successful BCI/FBI background check prior to employment and every 5 years.

Working Conditions                                                               

• Could engage in heavy lifting up to 50 lbs, stand for long periods, climb ladders, and work in awkward positions.
• Expected to complete tasks quickly to minimize downtime and disruption to operations.
• A strong understanding of mechanical, electrical, and plumbing systems, as well as knowledge of building codes and safety regulations, which might require continuous training and education. 
• Effective communication is essential for understanding work orders, coordinating with other team members, and providing updates to supervisors.
• Could face harsh working conditions, including extreme temperatures, exposure to hazardous materials, and work in confined spaces.
• Ability to handle multiple tasks simultaneously, prioritize urgent repairs, and meet deadlines, leading to stress and burnout if not managed effectively. 
• This position generally works in a variety of environments, using a variety of tools and equipment, both inside and outside residential and commercial facilities.
